Background: Niemann-Pick Disease (NPD) is a rare autosomal recessive disorder associated with intracellular deposition of sphingomyelin and cholesterol storage within the lysosome. There are mainly three types of NPD namely types A, B, and C. NPD type B is generally later in onset with a good prognosis for survival into adulthood and usually with no neurological abnormalities.
Case Presentation: The patient, male, was born in 35 weeks of pregnancy. Failure to thrive started at around 4 months and the diagnosis of type B Niemann-Pick disease was made approximately at the age of 2 years. Currently, the patient is 10 years old and having undergone 3 years of Traditional Persian Diet, has had obvious improvement in growth and laboratory tests.
Conclusion: The NPD is no specific treatment, especially in modern medicine. Considering the significant improvement in the patient’s condition after an appropriate diet and traditional medicines, it seems that traditional medicinal therapies have great results in the treatment of NPD. In this study, we presented an innovation for NPD treatment by the use of a Traditional Persian diet.

Background: NAFLD is the most common cause of chronic liver disease worldwide. The recommended therapy is lifestyle modification, which consists of dietary interventions and physical activity. The first line of treatment in Iranian Traditional Medicine (ITM) regardless of the type of disease is following the six essential principles named Setteh-ye- zarourieh, of which diet is the most important. The long process of treating NAFLD requires a comprehensive lifestyle design, which can be suited for the patients.
Case Presentation: A 57-year-old woman is a known case of NAFLD (grade II-III) accompanied by Insulin resistance, obesity and abnormal liver function tests (LFT). The accompanied sign and symptoms were heartburn, taste of acid, constipation and moderate weakness. The quality of life assessment was performed with SF-36 questionnaire.
Intervention: The patient underwent three months of dietary treatment based on Iranian Traditional Medicine (ITM).
Conclusion: Insulin resistance, obesity and LFT levels improved completely in 3 months and grading of fatty liver decreased to I-II. Afterwards the liver improved entirely and remains healthy after 2 years.

Background: Treatment of male infertility is an issue in all societies. Abnormalities of sperm parameters constitute almost half of the causes of infertility worldwide. The cause of the disorder is unclear in many cases, and thus male idiopathic infertility is diagnosed for the patient. Medications are usually not very successful in these patients. Therefore, many patients have to resort to assisted reproductive technologies at a high cost. Hence, finding effective therapies for clinicians and researchers is of great value.
Case Presentation: The patient is a 34-year-old man with a two-year history of idiopathic infertility. He had used various drugs to improve sperm parameters, none of which was successful. Finally, after 3 months of using an Iranian Traditional Medicine (ITM) product, lobub morakab, the patient had a significant improvement in spermogram parameters and the patient’s spouse became pregnant.
Conclusion: ITM offers various treatments and strategies for infertility, one of which is the drug reported in this article. Integrating the rich resources of ITM with assisted reproductive technology can bear good results for patients.

Henna or Lawsonia inermis L. is a plant used for medicinal and cosmetic purposes worldwide. Lawsone is an important chemical ingredient of henna leaves with an oxidizing effect that can cause acute haemolysis in vulnerable individuals including G6PD deficient patients. One of the symptoms of haemolysis is red discoloration of urine. It is thought that this effect of henna has been discovered by modern medicine but there are earlier reports in history of medicine. In fact, investigating traditional medicine books shows that urine color change due to topical use of henna has been mentioned by Aghili Khorasani who is a famous scholar of Iranian Traditional Medicine in the 12th century.
In parts of world where unexplained cases of hyperbilirubinemia and G6PD deficiency are more frequent, cutaneous application of henna has the potential to initiate oxidative haemolysis, which may present as red discoloration of urine. Since henna is widely used in different joyful events and also as a home remedy for minor problems, this effect should be considered..

Background: Umbilical pilonidal sinus (UPS) is caused by penetration of hair into the skin in the umbilical area. Patients may be asymptomatic at first and gradually present with pain, bleeding, pruritus, extravasations, periumbilical dermatitis, and granulation tissue. The standard treatment is surgery, which is aggressive with risk of various complications. Since there is no definite treatment for this disease, and considering the many side effects and complications of treatment options, and also the trend to use alternative and complementary medicine, this case study made an attempt to use Iranian Traditional Medicine (ITM) to treat umbilical pilonidal sinus.
Case Presentation: The patient was a 37-year-old man who presented with extravasation, redness, swelling, and stinging in the umbilical area. The patient had visited a surgeon, where he was advised to undergo surgery. The patient consented to using ITM for treatment and was prescribed Mamraghan poultice with which he was cured after two days of use.
Conclusion: Iranian Traditional Medicine uses various topical treatments to treat ulcers. We used a poultice recommended in PM references to heal and cure UPS.

Background: Gastroesophageal reflux (GER) is the most common disorder of childhood. GER can continue to older ages, reducing quality of life. Unfortunately, therapeutic interventions treatments are not always successful. It is the first report of pediatric GERD treatment in Iranian Traditional Medicine (ITM).
Case Presentation: An 8-year-old boy with recurrent GER and chronic abdominal pain was referred to a Traditional Medicine clinic. The child had suffered from severe irritability and recurrent regurgitation since infancy. Endoscopy was performed in the second year of life and severe esophagitis found. He was treated with oral omeprazole (1 mg/kg/day divided in 2 doses). After a month, abdominal pain and irritability continued. His problems remained until the age of five despite use of medication. Gastrointestinal bleeding (GIB) occurred in December 2013. In that month he was hospitalized twice times. In February 2014, he was admitted to the clinic of traditional medicine, due to poor feeding and abdominal pain. He was diagnosed with GER and stomach weakness. Myrtus communis (Myrtle) syrup 2.5 cc three times daily and quince jam for breakfast were prescribed. Abdominal pain improved after two months of treatment and within 2 years of follow-up his general condition was excellent. During this time there were no signs or symptoms of reflux recurrence.
Conclusion: Myrtle is useful in reducing edema, and strengthening the stomach and gastrointestinal tract. The effect of “quince” in the treatment of GERD is similar to omeprazole, while it also has other properties such as strengthening the LES. These fruits overcome the most common mechanisms of GER and have a direct role in the patient’s improvement.
