Low back pain can have various causes, one of which, usually disregarded, is the myofascial pain syndrome, which is caused by trigger points in the muscles and which leads to occasionally acute low back pain with radicular patterns. Timely and correct diagnosis of the cause of low back pain can help greatly in opting for the appropriate medical plan.
Case Presentation: The patient is a 34-year-old man suffering from chronic low back pain for the last four years. He had been afflicted by acute radicular pain in his left leg because of intense physical activity the day before seeking treatment. In the examinations conducted, the patient’s straight leg raise (SLR) test result was positive at 20 degrees, his left leg’s Achilles reflex was absent, and he could not stand on his left toes. Through palpation of the gluteal area, several trigger points were palpated and discovered in the gluteus muscles, especially gluteus medius and piriformis. Conservative treatment including resting, medicine, and physiotherapy was initiated, which improved the patient’s conditions to some extent Subsequently, a dry needling process was conducted for the gluteal region trigger points. After the sixth session of dry needling, the patient improved significantly, his SLR test was negative, and he returned to his normal work.
Conclusion: Attending to various causes of low back pain with a radicular pattern, including myofascial pain syndrome, can help greatly in selecting an appropriate treatment plan. In the cases where low back pain is accompanied by active trigger points in muscles, dry needling and injection can help avoid invasive and expensive treatments.https://www.ajtcam.ir/article_84981_342d177a0246823ab1b79241b982431d.pdfHajtaleb Medical Complex of Alternative and Complementary Medicine, Bojnurd, IranAsian Journal of Traditional, Complementary and Alternative Medicines2717-27831Issue 1-220181201Improvement in Sperm Quality and Reduction of DNA Fragmentation with Bee Pollen Supplementation: a Case Report7119010910.22040/ajtcam.2018.90109ENSepideh HesamiMSc. student. It is defined as the inability to conceive within one year of regular unprotected sexual intercourse. Some natural antioxidants can improve sperm quality and help infertile patients. We report a patient with idiopathic infertility who had been trying to conceive for eight years with no success.
Case Presentation: A 32-year-old non-smoker, overweight man, was referred from an urologist due to oligospermia. His infertility was diagnosed as idiopathic. He had been trying to have a child for eight years, had received antioxidant supplements for the first five years. Bee pollen in powder form was prepared and administered to the patient for two months. After intervention, all semen parameters except motility (grade d) increased. The inflammatory markers decreased due to supplementation. There was also an increase in testosterone level and a decrease in FSH, LH and prolactin levels following the eight-week treatment period. Moreover, sperm DNA fragmentation reduced from 32.4% to 12.7%.
Conclusion: Bee products have been used as medicinal substances since the ancient times. Bee pollen is rich in flavonoids, some of which have anti-inflammatory and antioxidant effects. In this study, we have shown that the spouse of a patient with idiopathic infertility can become pregnant after he received supplementation with bee pollen.https://www.ajtcam.ir/article_90109_61dbab765eeccfffc63cf8c36a749991.pdfHajtaleb Medical Complex of Alternative and Complementary Medicine, Bojnurd, IranAsian Journal of Traditional, Complementary and Alternative Medicines2717-27831Issue 1-220181201A Case Report of Chronic Allergic Rhinitis Treatment in A 29-Year-Old Woman Based on Iranian Traditional Medicine12218708810.22040/ajtcam.2018.87088ENMehrdad KarimiAssistant Professor, Department of Traditional Medicine, School of Persian Medicine, Tehran University of Medical Sciences, Tehran, IranSima KolahdoozMD, PhD in Persian Medicine, Department of Traditional Medicine, School of Persian Medicine, Tehran University of Medical Sciences, Tehran, IranJournal Article20181007Introduction: Allergic rhinitis is one of the most commonly diagnosed allergic diseases, the prevalence of which is increasing globally. The purpose of this case report is to outline the Iranian traditional medicine (ITM) approach to diagnosis and treatment of chronic allergic rhinitis.
Case presentation: A 29-year-old woman with a long history of symptoms of runny nose, sneezing and itchy throat was diagnosed with seasonal allergic rhinitis and treated with antihistamines. Since 9 years ago, the disease pattern changed from seasonal to a year-round state and the patient referred to the traditional medicine clinic due to inadequate response to conventional therapies and reluctance to consume oral corticosteroids. Initially, the six lifestyle essentials and prohibited foods were explained to the patient, followed by drug therapy to cleanse the head region and body and to eliminate the underlying causes of the disease. The post45-day treatment assessment showed a significant reduction in the severity and frequency of symptoms.
Conclusion: Clinical decision-making in ITM is based on a holistic view of the patient rather than the disease, which is the case in conventional medicine. This case report is an example of a patient-based treatment protocol in the Iranian medical system. The systemic approach of ITM in dealing with a patient with allergic rhinitis and simultaneous attention to brain and whole-body cleansing along with the six essentials of lifestyle modification resulted in significant improvement.https://www.ajtcam.ir/article_87088_b564abfe86baa9de39db2e78f208f6a4.pdfHajtaleb Medical Complex of Alternative and Complementary Medicine, Bojnurd, IranAsian Journal of Traditional, Complementary and Alternative Medicines2717-27831Issue 1-220181201Successful Management of Severe Mentally Retarded Child by Homeopathy22268709310.22040/ajtcam.2018.87093ENAhmad MahmoudianAssistant professor, Department of Community and Family Medicine, Faculty of medicine, Isfahan University of Medical Sciences, IranMonirosadat AminGeneral Physician, Member of (Liga Medicorum Homoeopathica Internationalis (LMHI), Isfahan, IranJournal Article20181016Background: There is no effective treatment for mental retardation, so only supportive care is provided for such patients. WCT was applied seven times. After the seventh session, the patient reported that five years ago he had been diagnosed with panic disorder and that he was on sertraline medication on a daily basis. After the first WCT session, he reported that his symptoms of panic disorder decreased, and after 15 days, he discontinued the medication and confirmed he did not experience any panic attack.
Conclusion: As evident in the current case, the effects of WCT on heart rate and the nervous system could play a role in the treatment of anxiety disorders.https://www.ajtcam.ir/article_87184_f2cb989433df6621a084bbb3695767f9.pdfHajtaleb Medical Complex of Alternative and Complementary Medicine, Bojnurd, IranAsian Journal of Traditional, Complementary and Alternative Medicines2717-27831Issue 1-220181201Reporting a Case of Nine-Year Resistant Toe Joint Pain Treated Based on Principles of Iranian Traditional Medicine32408718910.22040/ajtcam.2018.87189ENHamed Naeiji1 PhD student, Student Research Office, School of Persian Medicine, Shahid Beheshti University of Medical Sciences, Tehran, IranRoshanak MokaberinejadAssistant Professor, Department of Traditional Medicine, School of Traditional Medicine, Shahid Beheshti University of Medical Sciences, Tehran, IranVahid AliannezhadiPhD student, Student Research Office, School of Persian Medicine, Shahid Beheshti University of Medical Sciences, Tehran, IranJournal Article20181106Background: musculoskeletal problems are among the most common complaints in outpatient visits. Treatment of joint pain in Iranian traditional medicine includes eliminating the cause, cleansing the body ot phfootm or the probable substance, transforming humors, and tonifying organs to prevent relapse and accepting loss of substance.
Introducing the patients: The patient was a 32-year-old thin man, without abdominal obesity and a relatively dark complexion with a history of more than nine years of pain in the right toe, which initiated as occasional pains and persisted almost continuously for the past three years. In spite of numerous medical follow-ups and two surgical operations since 2012, his condition did not improve and the patients had to consume painkiller constantly, such that he finally was recommended to amputate the affected toe.
The patient’s pain at the time of visit was very severe. The pain was more severe at nights, which awakened the patient, and was stiff in the morning. Exertion of any strike (of any intensity) caused pain intensification in the joint. Further, in some cases the pain was followed by inflation, redness, and warmness of the joint.
In examination, inflammatory factors of the patient were negative. In bone scan, a lesion was reported in the first metatarsal bone of the right foot.
The patient was irritable, with rapid irritability and late relief along with symptoms of aggression, anger suppression and its resulting mental traffic, obsession (mental and occupational), meticulous, huge mental traffic, stress, anxiety, worry, and sensitivity to the surrounding noises.
Conclusion: Accumulation of black bile in the first metatarsal joint of the right foot caused development of severe pain in the patient’s foot. Through time, gradual condensation of the substance occured, making treatment more difficult. Accordingly, it was necessary that in addition to modifying the patient’s humors, the substance accumulated in the joint be transformed to facilitate its excretion. Therefore, pharmacological treatment, topical unction, and following measures along with benefiting from the Fateh traditional manipulative treatment and leech therapy were used. The overall outcome was diminished pain, such that over the past five months, the patient did not report any kind of painhttps://www.ajtcam.ir/article_87189_c2975d8623f265894cd38b05da34568f.pdf
